Description:

The purpose of the grant opportunity is to improve access to eye care for First Nations patients through improved coordination of services, including improving linkages between the range of eye health services required by patients and delivery of those services. This will allow for coordination of services across the full spectrum of eye care, including those delivered through the Medical Outreach Indigenous Chronic Disease Programme (MOICDP), Rural Health Outreach Fund (RHOF), the Visiting Optometrists Scheme (VOS) and the eye element of the Eye and Ear Surgical Support (EESS) program.

The planning and implementation of the CIEH program will be undertaken in partnership with eye health care stakeholders working in the jurisdiction, including Primary Health Networks, Local Hospital Networks/Districts, Aboriginal Community Controlled Health Services and their jurisdictional affiliates, eye health care service providers, state/territory health departments and local health organisations.

The main objective of the grant opportunity is to improve access to eye care for First Nations people through improved coordination of culturally appropriate and safe services.

The intended outcome of the grant opportunity is to improve integration of services from initial consultation to treatment, referral and continuity of care, to allow for streamlining and improving the care pathway for First Nations patients.

To be eligible, you must be one of the listed organisation/s

  • General Practice Queensland Limited
  • NSW Rural Doctors Network Ltd
  • Northern Territory of Australia
  • Rural Doctors Workforce Agency Incorporated
  • Rural Workforce Agency; Victoria Limited
  • The Crown in the Right of Tasmania as represented by the Department of Health
  • Rural Health West Ltd

The list of eligible applicants above was determined on the basis that they are the organisations currently funded to deliver the Coordination of Indigenous Eye Health, and have proven to have:

  • capability to deliver the grant activities
  • existing infrastructure and relationships to support the grant activities
  • experience and capability to deliver the grant objectives and outcomes and
  • demonstrated value for money in delivering outcomes in the current market.

 

The program addresses the need for coordination of services across the full spectrum of eye care for First Nations people. The existing jurisdictional fundholders have demonstrated significant expertise in delivering coordinated and integrated services through the Commonwealth’s outreach programs - RHOF, MOICDP, VOS and the eye component of the EESS. The Coordination of Indigenous Eye Health program complements the eye health services being delivered through these outreach programs and other existing eye care services, and has been funded by the department since 2015-16.

 

The fundholders have strong, long-standing relationships with First Nations health stakeholders, service providers and health professionals. They have well-established infrastructure and are agile and innovative in delivering integrated health solutions as part of culturally appropriate services in remote locations.
