Description:

The Indigenous Australians’ Health Programme (IAHP) was established by the Australian Government on 1 July 2014. The IAHP provides funding for a range of activities including those focusing on primary health care, child, maternal and family health, and chronic disease prevention and management. The IAHP is made available under Outcome 1: Health Policy, Access and Support, Program 1.3: First Nations Health from the Department of Health and Aged Care’s Portfolio Budget Statement.  

The objective of the IAHP is to increase Aboriginal and Torres Strait Islander people’s access to safe and effective essential health services. The expected outcomes of the IAHP include improvements in:  

  • the health of Aboriginal and Torres Strait Islander people 
  • access to high quality, comprehensive and culturally appropriate primary health care, and  
  • system level support to the Aboriginal and Torres Strait Islander primary health care sector to increase the effectiveness and efficiency of services.  

The Integrated Team Care (ITC) Program is funded under the IAHP. The ITC program was established in 2016-17 and aims to: 

  • improve health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through better access to coordinated and multidisciplinary care, and 
  • contribute to closing the gap in life expectancy by improving access to culturally appropriate mainstream primary care services for Aboriginal and Torres Strait Islander people.

Eligibility:

To be eligible to receive funding under this Grant Opportunity, you must be one of the listed organisations or PHNs.

The list of eligible applicants was determined in keeping with the intent of the program to provide a national network of regional organisations that commission and integrate services at the local level to address identified needs and provide support and system level integration for health services to improve coordination of care for patients.

$77,519,290.00
