The purpose of this grant opportunity is to enable current DSOA service coordinators and those who are eligible, to deliver select disability services to older Australians with a disability who are currently receiving DSOA funded supports and are unable to be supported by the in-home Aged Care system (Commonwealth Home Support Programme or Home Care Packages).

To be eligible for this grant opportunity, you must be an organisation who is either currently delivering DSOA funded services and/or has been assessed as suitable based on the client support needs and locality.

The organisation must be able to demonstrate that they are able to support the client during the duration of this grant period because the client has chosen them to support them to access the activities funded under the DSOA Program. The list of activities that are funded through DSOA are noted in the DSOA Program Manual.
