Description:

The Medical Outreach Indigenous Chronic Disease Program (MOICDP) helps to improve access to health services for First Nations people living with chronic disease. Under the MOICDP, multidisciplinary outreach clinical services are provided by a range of health professionals such as medical specialists, GPs, nurses and allied health professionals, for best practice chronic disease prevention, treatment, management and follow up. 

Access is increased by removing the disincentives incurred by health professionals providing outreach services (e.g., by funding their travel, meals, and accommodation). For the purposes of the program, a chronic disease is defined as 'a condition that has been, or is likely to be, present for six months or longer'.

The objectives of the program are to: 

  • increase access to multidisciplinary care in primary health care settings for First Nations people with chronic disease, and
  • increase the range of services offered by visiting health professionals to prevent, detect, and manage chronic disease more effectively.

The intended outcomes of the program are:

  • new outreach services established that focus primarily on management of chronic disease for First Nations people, and existing outreach services are expanded where required
  • improved services based on location of greatest need
  • improved health service planning
  • improved collaboration between health services in local First Nations communities and visiting health professionals to target the delivery of essential treatment for chronic disease
  • enhanced coordination of services
  • improved culturally appropriate care for First Nations people, and
  • reducing/removing the financial disincentives that create barriers to service provision to First Nations people.

Eligibility:

To be eligible, you must be one of the listed organisations:

  • General Practice Queensland Limited
  • Health Network Northern Territory Ltd
  • Rural Doctors Workforce Agency Incorporated
  • Rural Workforce Agency; Victoria Limited
  • The Crown in the Right of Tasmania as represented by the Department of Health
  • Rural Health West Ltd

The list of eligible applicants above was determined on the basis that they are the organisations currently funded to deliver the MOICDP and have proven to have:

  • experience and capability to deliver the grant opportunity objectives and outcomes
  • capacity to deliver the MOICDP grant activity, and
  • established infrastructure and relationships with First Nations communities, health service providers and other key stakeholders that will support them to deliver the MOICDP.

The program addresses the need for coordinated services to address chronic disease for First Nations people. The existing jurisdictional fundholders have demonstrated significant expertise in delivering coordinated and integrated services through the Commonwealth’s outreach programs – Rural Health Outreach Fund (RHOF), Visiting Optometrists Scheme (VOS), the Eye and Ear Surgical Support Program (EESS), and Healthy Ears – Better Hearing, Better Listening (HEBHBL). The MOICDP complements the health services being delivered through these outreach programs and other existing eye care services and has been funded by the department since 2015-16.

The fundholders have strong, long-standing relationships with First Nations health stakeholders, service providers and health professionals. They have well-established infrastructure and are agile and innovative in delivering integrated health solutions as part of culturally appropriate services in remote locations.
