Supporting Outreach Healthcare for Victim-Survivors of Family, Domestic and Sexual Violence Pilot
The grant opportunity will fund Primary Health Networks (PHNs) to pilot a place-based support model by establishing outreach care teams, to provide health care in crisis, temporary accommodation, and services to support women, children and young people experiencing FDSV.
Objectives:
- develop and implement an outreach model of support to improve quality of care for victim-survivors of FDSV.
- increase access to holistic and integrated outreach health care to support victim-survivors, including those from population groups at higher risk of FDSV.
- increase integration with health and community services address barriers to accessing health and support services
For this grant opportunity up to $5.748 million (GST exclusive) is available over two years from 2024-25 to 2025-26. This includes up to $110 thousand (GST exclusive) for the Hunter New England and Central Cost PHN under funding Stream 1 to support and mentor Stream 2 participants. Up to six grants will be awarded.
